  Quality Care Assessment Fund Update

The Quality Care Assessment Fund legislation was introduced in both the House and Senate and can be followed under HB2673 and SB546. The language is virtually the same in both versions. A hearing is scheduled for 10:30am on Thursday, February 25, 2010 in the Ways and Means Committee on the Senate side and on that same day at 3:30pm in the Aging and  Long-term Care Committee on the House side.

Your Legislators need to hear from you now - both in the House and the Senate. Tell them how important it is for them to support these bills.  Now is the time to tell about cut backs you have had to make because of the 10% Medicaid cuts. Now is the time to tell about person centered care projects that have been placed on hold. Now is the time to tell about staff cut backs and the ripple affect they are causing on the local economy. Use the bill number when you speak with them.

Now is also the time to remind the Legislators that HB2673 and SB546 provide the solution to our funding crisis without dipping into the already over burdened state budget. Please contact your Legislators and tell them your stories. Check the list below and see if your Representative or Senator is on one of these targeted committees. Your contact with them is even more important. You can go to the KHCA website and click on the Advocacy tab to find your legislator and send them a message. Need help?   New RAC information from CMS

CMS  has approved SNF consolidated billing as an issue for most of the states in Region D.
CMS has approved the audit issue below for the RAC in the following Region A states: MA, ME, NH, RI, VT.
These issues will more than likely be approved by CMS for all RAC Regions.
Providers should check the RAC issue posting sites on a regular basis:

Issue Name: Clinical Social Worker (CSW) Services
Issue Number: A000222009
Issue Description:  CSW services rendered during an inpatient acute care or skilled nursing facility stay are not separately payable under Medicare Part B, instead they are included in the facility’s Prospective Payment System (PPS) payment. CSW providers are expected to render services under arrangement with the facility. Therefore, an issue may exist when a beneficiary received CSW services during an inpatient stay, which have been billed and reimbursed under Medicare Part B.

  2010 Census Procedures Differ in AL and SNF

The 2010 Census is underway and AHCA/NCAL is working closely with the Census Bureau to make sure the process goes smoothly and eases any burden on our members.

The Census Bureau has developed special procedures to count individuals living in “group quarters;” including college residence halls, residential treatment centers, skilled nursing facilities, group homes, correctional facilities and workers’ dormitories. Nursing facilities and group homes clearly fall under group quarters.  In addition, Intermediate Care Facility for Individuals with Mental Retardation (ICFs/MR) may have been classified as a group home or school for people with disabilities.  See Form D-351(GQV) Group Quarters Validation Questionnaire and Form D-1028.4 (Flashcard).  The status of assisted living and residential care facilities, however, is less clear.

For group quarters, there are three phases: 1) validation (completed October 23), 2) advance visit (on-going through March 19), and 3) enumeration (beginning April 1st).(Overview of 2010 Census Group Quarters Operations)

During the validation phase, Census workers classified assisted living facilities, residential care facilities and ICFs/MR as either housing units or group quarters, depending on the response from a representative of the facility.  Facility representatives self identified each facility using Form D-1028.4 (Flashcard).  If an assisted living facility, residential care facility or ICF/MR was identified and classified as a “housing unit” then each resident in those settings will receive a 2010 Census questionnaire in the mail and will be asked to mail back the completed form; if completed forms are not mailed in, Census workers will follow up by conducting an in-person visit. This is the same procedure the Census Bureau uses for enumerating all residential households.

During the Group Quarters Advance Visit phase, which applies only to group quarters (including all nursing facilities and group homes), providers will have an opportunity to meet with the Census Bureau worker and discuss, understand, and prepare for enumeration.

During the Group Quarters Enumeration phase, a Census Bureau worker will accept “administrative enumeration” when counting group quarters if the facility representative determines that this is the best procedure to use for their residents.  In this case, the facility generates a list of all the individuals in the facility, and the Census worker will develop a system of patient identification numbers from that list.  If the individual in the facility is capable, they fill out the census form, Form D-21(ICR) Individual Census Report themselves.  An informational copy of this report is attached, (D-61(ICR) Informational CR.pdf).  If the individual is incapable, the enumerator completes the form using administrative records; not medical records.  No facility classified as a group quarters should feel compelled to use its own staff to complete the enumeration process.  If the facility chooses to self-enumerate, the Census Bureau will provide staff training.  Note:  Please do not photocopy the Individual Census Reports (ICRs).  Each ICR has a barcode that is unique.

When a Census worker arrives at your facility be aware of the following information:

• Ask to see the worker’s badge to ensure the worker is from the Census Bureau. (See Census badge)
• Rest assured that Census information is confidential and protected by law. 
• Census data is used only for statistical purposes and is public only after 72 years. (See d31_100308_Confidentiality Notice)
• None of the information requested by the Census Bureau is protected health care information under the Health Insurance Portability and Accountability Act (HIPAA). (See Facility Manager Access Letter__Health Care)

  March is National Nutrition Month

March is National Nutrition month and the theme is "Nutrition From the Ground Up."

National Nutrition Month® is a nutrition education and information campaign created annually in March by the American Dietetic Association. The campaign focuses attention on the importance of making informed food choices and developing sound eating and physical activity habits. Registered Dietitian Day, also celebrated in March, increases awareness of registered dietitians as the indispensable providers of food and nutrition services and recognizes RDs for their commitment to helping people enjoy healthy lives.

  "Dear Jon" an update on Jon Covault

Former KHCA Chairman and long time administrator at Golden Heights continues his battle with cancer. As many know, he has been very open about the struggles and trials this disease and on going treatment have been for him, his family and extended facility family.  Jon has been couragous enough to journal or "blog" about his experiences both on Facebook and Caringbridge.
